Medicare Benefit

Criteria for Eligibility:

• Demonstrated to be a considerable and taxing effort to leave home (i.e. homebound).

• A skilled need (nursing or therapy).

• A change of status such as a hospital stay, medication change, or new diagnosis.

• Under the care and authorized by a physician.

• A Plan of Care developed and implemented.

• Visits by skilled team must be intermittent.

Example of Medicare Benefit Combined with Non-Medical Home Care:

**Note: Non-medical home care (i.e. personal care, companionship, and home helper services) is not covered by Medicare.
